I tried to log in to my MyWellness Portal, but it said my record was not found. What do I do?

Newly admitted students may experience difficulty logging in to their MyWellness Portal and will need to wait until their registration information is reflected in Banner. Please ensure you have claimed your USDOne account. If you have already claimed your USDOne account, it will take 2-3 business days to access your MyWellness Portal.

How do I access my secure messages on my MyWellness Portal?

Before you can use Secure Messages, you must fulfill the compliance requirement. Select "Medical Clearances". Click on "USD Annual Notice of Privacy Practice & Consent to Treat" at the bottom of the page.

Read the electronic Consent and electronically check two boxes (one for Consent and one for Notice of Privacy Practices). Once completed, you can proceed to your messages by selecting "Messages" from the left menu bar.

Only currently enrolled students have access to secure messaging. For all other inquiries, contact the SHC during business hours at (619) 260-4595 or by email at usdhealthcenter@sandiego.edu. (NOTE: Our medical staff is unable to discuss or process medical information via email, as email is not reliably confidential.)

Contact SHC

How do I contact the Student Health Center medical staff?

Please log in to your MyWellness Portal and send all medical-related questions and/or medical records via secure message. Our medical staff is unable to discuss or process medical information or immunizations via email.

First-time users must review and acknowledge the Consent to Receive Services form and update their local phone number. After logging in, click "Messages" > "New Messages" and follow the prompts to send a message.

With Student Health Center secure messaging, you may:

  • Sign consent forms for treatment and release of medical information
  • Complete health history forms
  • Submit and print immunization records
  • Receive and send secure messages to Wellness providers
  • View upcoming appointments and cancel appointments
  • Review the Student Health Center account
Student Health Insurance Plan (SHIP) / Referrals

What is the difference between the Student Wellness Fee and the Student Health Insurance Plan (SHIP)?

The Student Wellness Fee paid by currently registered undergraduate, graduate, law, and paralegal students provides access to the on-campus Student Wellness services such as the SHC. It is not possible to waive this fee.

The SHIP premium charged to the Student Account is different from the Student Wellness Fee. Learn more about the coverage dates, premium account, and benefits.

I have questions about enrolling or waiving out of SHIP

The Student Health Center does not manage SHIP.

For questions regarding SHIP, please reach out to Gallagher Support at (877)241-4649. If you have already attempted to contact Gallagher Support, please email USD Wellness team for assistance at wellness@sandiego.edu. A team member will review your case and reply to you promptly.

Is dental and/or vision included in SHIP?

SHIP does not automatically include dental or vision. You would need to enroll in a dental or vision plan voluntarily at an additional cost. If you haven't already enrolled, you can do so by visiting Gallagher - Dental & Vision Insurance website to get a quote or enroll.

How do I obtain a referral from the SHC?

Please reach out to the SHC via your MyWellness Portal Messages.

Click Messages > New Message > I want to send a message to the Student Health Center > I want to request a Referral

***If you have Kaiser, Medi-Cal, or another HMO insurance plan, a referral from the SHC will not be accepted by your specialist. Please reach out to your PCP for a referral if you have one of these plans.

Canceling & Rescheduling Appointments and Arriving Late

How do I cancel and/or reschedule my appointment?

To CANCEL your scheduled appointment with the SHC, please call us at (619) 260-4595 during business hours. You can also cancel your appointment up to one hour before your scheduled appointment by logging into your MyWellness Portal. Click Appointments and scroll down to the bottom to find currently scheduled appointments.

To RESCHEDULE an appointment, please call us at (619)260-4595 during business hours. You can also schedule a new appointment through your MyWellness Portal by clicking the Appointments tab. Please remember to cancel any of your currently scheduled appointments if you are rescheduling an appointment. (See above for how to cancel an appointment)

What if I am running late or show up late to my appointment?

Please note patients unable to arrive on time for their scheduled appointment will be rescheduled. If you know you're running late, call the front desk ASAP at (619) 260-4595 to let us know and we may be able to accommodate you.

A no-show fee of $40 WILL be assessed for missed specialty appointments (initial ADHD semester eval, travel consultation, sports physicals).

Billing and SHC Charges

Do I have to pay any fees to be seen at the SHC?

All students who paid the Student Wellness fee are eligible for services. Most primary care examinations are free of charge through the collection of the Student Wellness Fee. There is no co-pay and no deductible.

There are reasonable fees charged for prescriptions and over-the-counter medications, laboratory tests, procedures, treatments, and specialty care services. You can view current SHC fees here.

I have questions regarding a bill and/or SHC charge. Who do I contact?

Please utilize your MyWellness Portal MESSAGES to communicate with our Billing Coordinator.

Requesting Medical Records

To request medical records (i.e. immunization records only, labs, x-rays, EKG reports, or all records) for personal use or when sending to a 3rd party, OR to discuss your treatment and/or condition with a Parent, Athletic Trainer, or other party, please complete an Authorization to Release Patient Information.

Currently enrolled students -- please complete the Authorization to Release Patient Information form through your MyWellness Portal. Click Messages > New Messages > I want to send a message to the SHC > I want to request Authorization to Release Patient Information. Sign a release for a third party to receive my health information and/or send/ request medical records.

Alumni/Former students -- please complete the following Authorization to Release Patient Information form and return it via email to usdhealthcenter@sandiego.edu or fax to (619) 260-2375

For questions about Medical Records, please email us at usdhealthcenter@sandiego.edu. This is not a secure email, please do not include any personal protected health information.

Please allow 72 hours for all medical records requests to be processed.

NOTE: We are not able to send medical records via email. Your options to receive records include mail, fax, or pick up in person at the Student Health Center.

Minor Parental Consent Form

Where can I find the Minor Parental Consent Form?

The Parental Consent to Receive Services for Minors form can be found in multiple locations.

  1. Parents with minors can locate the form on the SHC website under the Forms tab.
  2. Students can log into their MyWellness Portal. Click the tab Medical Clearances > Consent to Treat Minor > Update > Download, complete, and upload the form > Save

How do I upload the Minor Parental Consent Form on the MyWellness Portal?

Log into your MyWellness Portal. Click the tab Medical Clearances > Consent to Treat Minor > Update > Download, complete, and upload the form > Save.

NOTE: There are two pages and sections that require both the student and parent(s)/legal guardian(s) signatures.

Update Personal Information

How do I update my personal information (i.e. preferred name, personal pronouns, gender identification, or local address)?

Personal information can be updated via your MySanDiego portal. Go to Applications > Select the option you want to update.

Medication Storage

My medication needs to be refrigerated, and I don't have a fridge in my dorm. Can I store it at the SHC?

Starting Spring 2024, students can store medication that needs to be refrigerated at the SHC for a flat fee of $50 a semester.

The medication would be stored in a temperature-monitored refrigerator to ensure the medication stays in the appropriate temperature range. The SHC would be responsible for any medication losses due to refrigerator malfunctions or mishandling.

Students will be responsible for dropping off and picking up medication during our normal business hours and will need to sign paperwork at each interaction to track inventory. Once the medication is checked out of storage, the SHC is no longer responsible for the medication. The SHC will NOT accept medication shipped through the mail or other mail delivery services.
