The Student Health Center medical staff can provide sick notes upon request.
It is up to the professor to excuse students from class or to make other accommodations. This wording is standard on all our sick notes and is a requirement that was put forth by the University's Faculty Senate. Most professors are very understanding and have been accepting of the sick notes. If you are having difficulty with a professor accepting a sick note, you can always discuss this issue with your dean.If you were NOT seen at the SHC for this illness, you can receive a "Reported Illness" note that states the date(s) you were ill. This type of note does clarify that you were not seen at the SHC.

You also have 24/7 access to medical and medical virtual health care ( Timely Care ). You can request a sick note during your TimelyCare visit.
If you were seen by an off-campus or telehealth provider, please attach documentation (take a photo or screenshot) of your after-visit summary to your MyWellness Portal message.

If you were seen at the SHC for an illness, contact the provider who treated you, via your MyWellness Portal Messages for a more specific note.
