What is the difference between the Student Wellness Fee and the Student Health Insurance Plan (SHIP)?
The Student Wellness Fee paid by currently registered undergraduate, graduate, law, and paralegal students provides access to the on-campus Student Wellness services such as the SHC. It is not possible to waive this fee.
The SHIP premium charged to the Student Account is different from the Student Wellness Fee. Learn more about the coverage dates, premium account, and benefits.
I have questions about enrolling or waiving out of SHIP
The Student Health Center does not manage SHIP.

Is dental and/or vision included in SHIP?
SHIP does not automatically include dental or vision. You would need to enroll in a dental or vision plan voluntarily at an additional cost. If you haven't already enrolled, you can do so by visiting Gallagher - Dental & Vision Insurance website to get a quote or enroll.
How do I obtain a referral from the SHC?
Please reach out to the SHC via your MyWellness Portal Messages.
Click Messages > New Message > I want to send a message to the Student Health Center > I want to request a Referral
***If you have Kaiser, Medi-Cal, or another HMO insurance plan, a referral from the SHC will not be accepted by your specialist. Please reach out to your PCP for a referral if you have one of these plans.
Do I have to pay any fees to be seen at the SHC?
All students who paid the Student Wellness fee are eligible for services. Most primary care examinations are free of charge through the collection of the Student Wellness Fee. There is no co-pay and no deductible.
There are reasonable fees charged for prescriptions and over-the-counter medications, laboratory tests, procedures, treatments, and specialty care services. You can view current SHC fees here.
I have questions regarding a bill and/or SHC charge. Who do I contact?
Please utilize your MyWellness Portal MESSAGES to communicate with our Billing Coordinator.

I have been sick, how do I get a note for my professors?
Please log into your MyWellness Portal. Click Messages > New Messages > I want to send a message to the SHC > Continue > I want to send a secure message to request a sick note.
- If you were NOT seen at the SHC for this illness, you can receive a "Reported Illness" note that states the date(s) you were ill. This type of note does clarify that you were not seen at the SHC. It is up to your professor to excuse the absence.
- If you were seen by an off-campus or telehealth provider, please attach documentation (take a photo or screenshot) of your after-visit summary to your MyWellness Portal message.
- If you were seen at the SHC for the illness, reach out to the provider that treated you, via your MyWellness Portal Messages for a more specific note.
To request medical records (i.e. immunization records only, labs, x-rays, EKG reports, or all records) for personal use or when sending to a 3rd party, OR to discuss your treatment and/or condition with a Parent, Athletic Trainer, or other party, please complete an Authorization to Release Patient Information.
Currently enrolled students -- please complete the Authorization to Release Patient Information form through your MyWellness Portal. Click Messages > New Messages > I want to send a message to the SHC > I want to request Authorization to Release Patient Information. Sign a release for a third party to receive my health information and/or send/ request medical records.
Alumni/Former students -- please complete the following Authorization to Release Patient Information form and return it via email to usdhealthcenter@sandiego.edu or fax to (619) 260-2375

Please allow 72 hours for all medical records requests to be processed.
NOTE: We are not able to send medical records via email. Your options to receive records include mail, fax, or pick up in person at the Student Health Center.
Where can I find the Minor Parental Consent Form?
The Parental Consent to Receive Services for Minors form can be found in multiple locations.
- Parents with minors can locate the form on the SHC website under the Forms tab.
- Emails are also sent to students before the beginning of each semester with the subject "URGENT: Parental Consent for Minors to Receive Services form" with instructions to forward the email to parent(s) or legal guardian(s).
- Students can log into their MyWellness Portal. Click the tab Medical Clearances > Consent to Treat Minor > Update > Download, complete, and upload the form > Save

How do I update my personal information (i.e. preferred name, personal pronouns, gender identification, or local address)?
Personal information can be updated via your MySanDiego portal. Go to Applications > Select the option you want to update.

My medication needs to be refrigerated and I don't have a fridge in my dorm. Can I store it at the SHC?
Starting Spring 2024, students can store medication that needs to be refrigerated at the SHC for a flat fee of $50 a semester.
The medication would be stored in a temperature-monitored refrigerator to ensure the medication stays in the appropriate temperature range. The SHC would be responsible for any medication losses due to refrigerator malfunctions or mishandling.
Students will be responsible for dropping off and picking up medication during our normal business hours and will need to sign paperwork at each interaction to track inventory. Once the medication is checked out of storage, the SHC is no longer responsible for the medication. The SHC will NOT accept medication shipped through the mail or other mail delivery services.
