Sports Physicals

Full physical exams are available for clearance to participate in Sport Clubs or other sporting activities. Upon medical clearance, a signed authorization to participate in sports letter will be provided.

NOTE: Sport Physicals are only available at the SHC for a limited time at the beginning of the Fall and Spring semesters.


To request an appointment, you must complete and submit the NEW Member Medical Packet prior to scheduling a “Sport Physical (Club Sport)”

  1. Log in to your MyWellness Portal and click the Downloadable Forms tab to complete and upload your New Member Medical Packet.
  2. Once you have completed and uploaded the form, your paperwork will be reviewed by SHC medical staff. Please allow 24-48 business hours for review.
  3. After your paperwork has been reviewed and approved, you will receive a message within your MyWellness Portal (Messages tab) with an appointment scheduling link and directions on how to schedule your club sport physical.

**Do not upload paperwork to DSE website until packet is completed by a medical provider.**

IMPORTANT REMINDERS

  • On the day of your appt., please arrive 10 mins before your scheduled time and allow about 30 mins for the appt.
  • If you have a prescription for glasses or contacts, please bring them with you.
  • There is a $50 charge for the Sports Physical, as it is a specialty service. This is payable on the day of service.
  • If you fail to show up at your scheduled appt. time, you will still be charged the $50, and you will need to reschedule the appt. and incur another $50 charge.
  • If you anticipate you’re running late, please call the SHC at (619) 260-4595 to reschedule and avoid being charged $50 for missing your appointment time.
  • Cancellations can be made through your MyWellness Portal up to 1 hour prior to your appointment, or call the SHC at (619)260-4595.

I Completed my Sport Club Physical at the SHC...Now what?

When you complete your sport club physical at the SHC, our staff will upload your completed forms to your MyWellness Portal. You will receive a secure message with your completed forms by the end of the day.

Please follow the directions provided by USD Sport Clubs (p.1) to submit your new member medical packet and insurance card to the USD Sport Clubs- DSE website. (It is recommended to use a laptop or desktop to download and submit your completed form to the DSE website)
