USD Student Health Insurance Waiver Requirements

If you are covered by a comprehensive insurance plan meeting the University’s requirements, you may submit a request to waive or opt-out of the USD Student Health Insurance Plan. Gallagher Student Health will review your waiver request and send approval or denial to your USD email address.

Students: please read the following requirements for waiving USD Student Health Insurance Plan

Your comprehensive health insurance plan must meet the University’s general requirements listed below. Please note that additional criteria may also be necessary in order for your waiver to be approved.

General Criteria:

*Provide continuous year-round coverage while you are a student at the University of San Diego.

  • Fully compliant with all aspects of USD waiver requirements and all federal/state mandates.
  • Underwritten and administered in the United States.
  • Access to local doctors, specialists, hospitals and other healthcare providers near campus.
  • Provides coverage for urgent and non-urgent care including,
    • preventative and routine benefits;
    • In-patient and out-patient surgery and hospitalization;
    • lab work, diagnostic x-rays, physical therapy, chiropractic care, emergency room treatment, ambulance services and prescriptions.
    • In-patient and out-patient mental health, substance abuse and counseling services.
  • I understand if I am covered by an out-of-state HMO or out-of-state Medicaid plan to waive, these plans typically cover only emergency and urgent care when applied outside of the state in which the plan/policy was issued.
    • Coverage for routine services, as well as treatment for ongoing conditions, will be excluded from coverage.
    • Consequently, the University strongly encourages students in these categories to purchase SHIP as a way to ensure comprehensive coverage, especially for students with chronic conditions or medical diagnoses requiring greater care.
  • I understand that I am waiving for this policy year only and I will need to waive coverage every year that I meet my school’s insurance eligibility requirements.
  • My health insurance covers me throughout the entire policy year.
  • I acknowledge that by waiving the student health insurance plan that I will be responsible for any medical expenses I incur.
  • I understand if I am an International Student, I may not waive the Student Health Insurance Plan unless I am covered by an insurance plan based in the United States and have met all noted criteria.

Upon request, you must be able to provide a copy of:

  • Verifiable proof of coverage with student’s name (Certificate of coverage, insurance policy or letter from the insurance carrier.)
  • Plan document(s) in English, with currency amounts converted to U.S. dollars, and the insurance company contact phone number in the U.S. is mandatory. All claims must be processed in the U.S. and paid directly to U.S. providers.

Some reasons why you may not be granted a waiver:

  • Your plan does not cover preventive care services at 100%
    • Your coverage only provides emergency/urgent care in your location of study;
    • or Your coverage may be a Healthcare Sharing Program which is not a contract for insurance nor a comprehensive insurance plan.
  • Your plan has a pre-existing condition exclusion.
  • Your plan has a per-injury or per-illness maximum benefit limits.
  • Your plan does not cover medical services for injury from participation in all types of recreational activities or amateur sports.
  • Students with coverage from insurance carriers based outside the United States.
  • International student insurance plans are not filed in the US and are not eligible for a waiver.
