Services Include

  • Up-to-date, accurate itinerary-specific health and safety advice for students traveling to 3 or fewer countries (for those traveling to 4 or more countries, a travel clinic using a travel support platform is more appropriate).
  • Routine immunizations as well as some travel immunizations. (NOTE: We no longer have the yellow fever vaccine available at the clinic, but we can give a prescription to get it at a pharmacy)
  • Prophylaxis for malaria and traveler's diarrhea, if indicated
  • Tuberculosis (TB) screening
  • Post-travel assessment at no extra charge

There is a $50 charge, payable on the day of service, for travel physical visits at the Student Health Center. Visits are limited to currently registered students. Further charges apply for medications, immunizations, and TB screening if needed. No-shows for scheduled travel appointments will be charged the full fee.

Pre-Travel Questionnaire

Prior to scheduling a Travel physical appointment, you must submit a completed electronic Pre-Travel Questionnaire.

How to Complete a Pre-Travel Questionnaire:

  1. Log on to your MyWellness Portal.
  2. Click the Messages tab and follow the prompts to request a Pre-Travel Questionnaire.
  3. Messages > New Message > I want to send a message to the Student Health Center > I want to request a Pre-Travel Questionnaire.
  4. If you need paperwork signed by a medical professional, please attach the paperwork to the message by clicking Add Attachment.

Please allow staff 24-48 business hours to respond to your request. Once your request is received and reviewed, a Pre-Travel Questionnaire will be sent to your MyWellness Portal. You can find the questionnaire under "Survey Forms". Once completed, please Contact Us to schedule.

A current copy of your immunization record, including any prior travel vaccines, is helpful but not mandatory.
