Apply for IRB Review

Once you have completed all steps on the Getting Started page, complete the following to apply.

Step 1: Contact your Faculty Advisor/Capstone Chair

  • The preparation of an IRB application requires careful planning and dialogue with IRB personnel. Researchers should first consult their Faculty advisor or Capstone Chair with any questions. Find your respective IRB representative on our Members page.

Step 2: Prepare Materials: Forms and Templates

  • Templates for all forms attached to an IRB application can be found on the Forms page. All document attachments must be in PDF format for archival purposes.
  • PIs are strongly encouraged to follow the provided templates when preparing documents for their applications. Some of the prescribed language may be modified to suit the specific activities called out in the proposed study. However, the PIs should keep in mind the federal and USD IRB compliance requirements and ethical principles for protections of human subjects when modifying the language.

Step 3: Create and complete application in Cayuse


Apply through the Cayuse IRB platform. PIs are encouraged to use the following step-by-step videos and instructions as a guide when filling out the application.
