ROTC Physicals

1. Log on to your MyWellness Portal to upload the following documents
   • High Risk Form - completed with your information
   • Completed ROTC Medical Exam (DD 2808) from DoD MERB
2. Click the Messages tab > I want to send a message to the Student Health Center > I want to submit my ROTC Physical Form. You will be able to attach the documents to the message.
3. SHC medical staff will review your paperwork and any information in your health center medical record chart. If there are no concerns, we will sign the paperwork and send it back to you via the secure message system. If there are any concerns, a medical provider may message you for clarification or request an in-person physical.

Please allow 24-48 business hours to process.

   • Completed ROTC Medical Exam (DD 2808) from DoD MERB

The Student Health Center is unable to complete the following forms:

• Aviation-related physicals
• Commissioning Forms
• Report of Medical Examination.
