Accounting Forms and Guidelines

Below are the most frequently requested accounting forms:

Journal Entry Request Form

Please use the Workday forms found here. In the Workday system search bar, type "Create Request". In the Request Type box, choose "All" and then choose "Journal Entry Request".

Worktag Request/Change Forms

This form is used to request a New Gift Worktag or Change a Gift Worktag in Workday. The Gift Worktag is used to administer the accounting of donor imposed contributions. Use the appropriate Gift Worktag Request or Change Form to request a new spendable or pooled gift in the Workday system. Type "Create Request" in the Workday search bar. In the Request Type box, choose "All" and then choose either "Change in Gift Worktag Request" or "New Gift Worktag Request".
