Personal Information

Citizenship Information

  • For U.S. Citizenship Status, indicate “None.”
  • For Country of Citizenship, if no citizenship is held, indicate your country of origin.
  • For Legal State/County of Residence, indicate your state and county of residency, regardless of the legality.
  • For Do you have a U.S. Visa?, indicate “No.”

Program Materials

Personal Statement

Please know that you are not required to disclose your status in your personal statement or any other part of the application or admissions process. It’s up to you if you would like to use your personal statement to discuss your status. Applicants who choose to do this typically do so to help reviewers understand their circumstances. If you choose to disclose, it will not be used outside the application process.

Request an Application Fee Waiver

If USD's application fee poses a financial hardship, you may be eligible for a waiver. If you are interested in an application fee waiver, please reach out to the School of Nursing (nursing@sandiego.edu) to request a waiver.
