I, the undersigned, am aware that:

If this lost or stolen parking permit is found on a vehicle, the vehicle and the permit will be impounded by the University of San Diego Department of Public Safety and impound fees will be paid by the Owner/Operator of the vehicle with the permit attached.

If the parking permit is found by the undersigned, it will be returned to the Department of Public Safety/Parking Services Division.

Any citations issued to the permit, prior to this date, are the responsibility of the undersigned.

The Department of Public Safety will investigate any claim of a permit that was reported stolen.  If an offender is caught in possession of a stolen permit they may be fined and/or prosecuted for the offense.

You must go to the Parking Services office in Hughes 150 after submitting this form to pay the replacement fine.
