MyStudentBody Alcohol Course, Part 2 Follow up Survey

DEADLINE EXTENDED to Monday, October 26th, 2009

The follow up portion of the MyStudentBody alcohol course must be accessed through an e-mail notification sent to each individual student approximately 60 days after completion of the MyStudentBody.com alcohol course.  This final portion of the program consists of a short survey that should take approximately 10 minutes to complete.

If you have not yet received e-mail notification for the follow up survey and it has been 60 days or more since you completed the initial course, or if you are experiencing technical problems with the follow up survey, please e-mail techsupport@mystudentbody.com for assistance. If you do not receive resolution within 48 hours of your e-mail request, please contact the Center for Health and Wellness Promotion via e-mail at chwp@sandiego.edu or call 619.260.4618 for further follow up.
