Forms

  • Direct Deposit – This form is used to sign up for direct deposit, make a change, or cancel your direct deposit.  Complete this form with required account documentation and bring to Payroll, Maher 106.  Click here for the form.
  • Change of Address & Paycheck Distribution form – This form is used to update your address or make a change to the paycheck distribution of your check.  Click here for the form
  • Make-Up Time Form – The Personal Obligation Make-Up Time Form is used when an employee requires personal time off but will not be using sick or vacation time during the absence.  This form must be completed and approved by a supervisor before the time is actually taken.  Time made up must take place in the same work week as the time taken off.  A copy of this form should be submitted to Payroll on the timecard due date.
  • Meal Break Waiver -- Waiving a meal break to allow an employee to complete their day's work may only be done when the employee's work period is not more than six hours.  The meal period may be waived by mutual consent of the employee and the employee's supervisor.  A copy of the Meal Break Waiver should be submitted to Payroll on or before the timecard due date.
  • Request for Copy of W2 – This form is used to request a copy of your W2 for a past year.  Click here for the form
  • Form W4 2008 Employee’s Withholding Allowance Certificate – This form is used to establish your withholding allowances for Federal income taxes.  If claiming exempt from Federal Withholding, you will need to complete a new form each year as your “Exempt” status expires February 15th.  Click here for the form.   • Form DE4 Employee’s Withholding Allowance Certificate – This form is used to establish your withholding allowances for State income taxes.  Filing this form is optional; however, if you want the same allowance to by applied to the State of California taxes write "Both" on the top of the Form W4.  Click here for the form
  • Form W5 2008 Earned Income Credit Advance Payment Certificate – The EIC is a special credit that reduces the amount of tax you owe (if any). There are four criteria’s you must meet in order to be eligible.  Please refer to the form under “Who is Eligible to get Advance EIC Payments?”  You must complete a new form each year that you are eligible as EIC form expires on December 31st.  Click here for the form.
