FERPA – Family Education Rights and Privacy Act of 1974
With this form, students may grant the University permission to release information about their records to a third party (including parents, step-parents, etc.) by submitting a completed Student Information Release Authorization. Please notice, you must complete a separate entry for each parent, family member, or other individual to whom you wish to grant access to information regarding your student records.
Title IV - Title IV Authorization form
Authorizes USD to pay miscellaneous charges on a student’s account with credits remaining from Title IV financial aid, including Federal Pell Grant, Federal SEOG, Academic Competitiveness Grant, National Smart Grant, TEACH Grant, Federal Perkins Loan, Federal Subsidized Stafford Loan, Federal Unsubsidized Stafford Loan, and Federal PLUS loans. Without this authorization, miscellaneous charges will not be paid and an enrollment hold will be placed on future registration.
The prepayment plan allows a discount of 1/3 % per month for payment in advance of all tuition, housing and fees. To receive the maximum discount available (1/3 % x 9 months x the calculated annual payment) the entire academic year needs to be paid by May 1. To receive the maximum discount by semester, fall term must be paid in full by May 1st and spring term must be paid in full by October 1st.
