Taking a Leave of Absence

Your Responsibilities

 Meet with HR/Benefits prior to the beginning of leave when possible.

 Provide your department with a written request for medical leave in advance of the beginning of the leave when foreseeable. The request should include leave start date and expected return to work date. If leave is not foreseeable, please notify the department immediately following the beginning of the leave. Your department will submit a DAF to Human Resources.

 Provide your department and HR with a copy of your doctor’s certification of disability which indicates the expected dates of disability and anticipated return to work date. This certification should be provided before your leave begins if foreseeable, or immediately after the beginning of your leave if unforeseen.

 Provide HR with a copy of your SDI earnings calculation so your salary can be properly integrated with the state benefit while you are on medical leave.

 Provide your department and HR with a copy of your doctor’s release to return to work. This note should indicate if you are released to full duty, or if you have work restrictions, what specific restrictions you have, and how long those restrictions are expected to remain in effect. If you are released to work with restrictions, you will need to provide another note when your doctor releases you to full duty.
