Spring/Summer 2019 Waive Cycle

The deadline to enroll in or waive out of the student health insurance plan for new students for Spring 2019 was March 1, 2019. If you did not submit a form to waive out by the deadline you have been enrolled in the plan and are responsible for the premium charged to your Student Account.

If there were extenuating circumstances that prevented you from meeting the waiver deadline, you can submit a Petition to Waive form online from March 2, 2019 - March 29, 2019.

  • The Petition to Waive process acts as a second level appeal for all full-time students who missed the published waiver deadline. Submission of a petition to waive does NOT guarantee approval.
  • Gallagher Student Health will email you their decision. Please ensure to submit to them any follow-up documentation that they request as soon as possible.
  • Click here to submit the online Petition to Waive (PTW) form to appeal enrollment before March 29, 2019
