In order to ensure the provision of reasonable and appropriate accommodations and services to students with disabilities, students must provide current documentation of their disability on their provider's letterhead.
Documentation must include:
- a clear diagnostic statement of their disability with diagnostic codes and description
- a description of the manner in which the disability limits the student, the severity and timeframe of the limitation
- test results (if applicable)
- recommendations for accommodations, adaptive devices and/or assistive services
Confidentiality
The Disability and Learning Difference Resource Center will maintain the confidentiality of these diagnostic reports to the extent permitted by law and will not release any documentation without a student's informed written consent.
