Online Forms
Web Services provides web application development support for USD.
This means we can take any HTML/PHP form and connect it to a database (MySQL). This makes the data and information collection process for departments much easier, since responses acquired via your form automatically go into a database rather than to an e-mail.
Development Process
- Build a Web form. This can be done in one of two ways:
- Ask us to build it for you. To get started, send an example of the form in Word to websupport@sandiego.edu so we can see what fields you need on your form.
- Build it yourself and then let us know the URL. You must build this form somewhere on the www server.
- We will connect to your site on www. If we need connection information from you, we will contact you.
- A MySQL database will be connected to your form.
- We will provide you with a method to access the information that is collected
via your form. This is done in several ways:
- A downloadable Excel spreadsheet; you will be given the URL where you can download this for collecting your data.
- Responses submitted via your form will also be sent via e-mail to a person/persons of your choice. Please provide us with the e-mail address(es).
For your security, any form will automatically have a single sign-on login screen so that only members of the university can access your form. This prevents spamming via your form. If your form is intended for external audiences, please let us know.
Time Frame
Turnaround time for web application forms is normally 2 weeks. However this greatly depends on the following:
- student staffing within Web Services
- the simplicity/complexity of your form
- any special requests that might go along with your specific needs
- how many changes you request

E-Commerce
If your form is collecting information for an event, class or other paid expense here at the university, we can connect it to a secure online payment area for credit card processing.
