Post-completion OPT Instructions

The deadline for Spring 2007 graduates to apply for post-completion OPT is Friday, May 11th, 2007. Please contact our office for an OPT appointment prior to this date and bring your completed application to the appointment.

Application Materials

• 2 color photographs (click here for instructions)
• Check or money order in the amount of $180.00 made payable to The Department of Homeland Security
• I-765 "Application for Employment Authorization" (page 10* only)
• I-538 "Certification by Designated School Official" (page 1** only)
• Current and previously issued I-20(s)
• Passport and white I-94 Arrival/Departure card

* Additional Instructions for I-765

• Item 10 refers to the 11-digit number on the top of your I-94 Arrival/Departure card
• Item 13 refers to the city where you landed and were inspected by U.S. Customs on your most recent trip back into the U.S.
• Item 16: complete the parenthesis as follows: (c) (3) (i)

** Additional Instructions for I-538

• Section A Item 3 refers to the 11-digit number on the top of your I-94 Arrival/Departure card
• Section A Item 4 refers to the date when you were granted the F-1 student visa for the first time.
• Section A Item 7: The beginning date is the first day you will work. The ending date is exactly one year later.
