Applications for Faculty-led Programs
To apply to an Intersession or summer abroad program, all students must complete an Intersession/Summer Abroad Application (signed by the appropriate program director) along with the Agreement & Release Form. All students must complete the checklist on the front of the application. Incomplete applications will NOT be accepted. Applications and all required materials must be turned in to the International Studies Abroad Office located in Serra 315.

FALL 2008 SEMESTER Preliminary Applications
The USD International Studies Abroad Office requires a preliminary application and several other documents to complete the registration process for a semester abroad. Students must submit all required materials indicated on the preliminary application and fill out an Agreement and Release form in order to be eligible for consideration for a USD-affiliated International Studies Abroad Program.

FALL 2008 USD/ITESO SEMESTER EXCHANGE Application
Students interested in participating in the USD/ITESO Exchange must fill-out a preliminary application along with an Agreement and Release Form. When completed, all materials must be turned in to the International Studies Abroad Programs Office located in Serra 315.

Power of Attorney Form
The Family Educational Rights and Privacy Act (FERPA) is a Federal law that protects the privacy of student education records. For this reason, personal student information may not be shared with friends, family, other offices at USD, or the host institution without legal permission from the student (applicable if student is 18 or older).
The USD Study Abroad Power of Attorney Form is not required though is highly recommended for study abroad students. This form allows a specified agent to discuss the student's: record, financial aid, and account information. This power of attorney is for a limited time only and is valid for the duration of the student's travel abroad. Upon return, the policy becomes null and void. Once completed, this form must be notarized, thereby become legally binding. Notary services are available in the International Center. Student Information Release Authorization
Students may also wish to file a FERPA Student Information Release Authorization with the USD Office of the Registrar. This allows the student to give a specified individual access to his/her billing, financial aid, and academic information.

Refund Deposit & Release Form
This form allows study abroad students to have his/her financial aid award: deposited directly into a specified bank account, sent by mail or made payable to a named agent for pick-up on the student's behalf. This form should be submitted directly to the Office of Student Accounts, located in the Hughes Center, Room 205.

Transfer Evaluation Form
When a USD affiliated program offers a course that does not appear on the USD list of approved courses (see course listings), please contact an International Studies Abroad Advisor (see Transfer Evaluation process) in the USD International Center for help completing a transfer evaluation form. Please note, this process may take up to a couple of weeks.

This form is required of all students participating in a USD study abroad program.

Internship Abroad Approval Form
Students interested in participating in an internship through the program abroad must read through the internship approval procedure and fill out the Internship Abroad Approval Form. All requirements must be met for credit to be granted.
