Enrollment in the USD Domestic Student Plan is NOT automatic; please follow the instructions below to enroll.

• Log onto www.GallagherKoster.com/USD
• Select the "Student Direct Pay" tab
• New users will be required to create a unique User Account.  Returning users will simply log in
• Complete the online enrollment form

Students have thirty days from the end of the semester to enroll.  Coverage will be effective the date the enrollment and premium is received.

(Note:  This insurance plan is not the USD Student Health Fee that is required of all USD students which provides access eligibility to USD Wellness Services at the Student Health Center, Counseling Services, Disability Services and Center for Health and Wellness Promotion during the fall and spring semesters, and if registered for intersession classes.)
