Required Immunizations and Screenings
ALL new students, including freshmen, transfer, and international students, and ALL residential students and staff must be immunized or screened for the following diseases:
- Hepatitis B (3 doses required; must have started the series, or have blood test indicating immunity)
- MMR (Measles, Mumps and Rubella) (2 doses required; must have started the series, or have blood test indicating immunity)
- Tetanus, Diphtheria, Pertussis (primary series administered during childhood with a booster within the last 10 years)
- Tuberculosis Screening (must have been administered within the past year)
- Varicella (Chickenpox) (must have had the disease or vaccination, or have blood test indicating immunity)
To satisfy this requirement, students and staff in the groups mentioned above must provide verification of their required immunizations and screenings, before the start of classes, by submitting the Student Health Information Form to the Student Health Center. Please submit your completed and doctor-verified immunization form to the Student Health Center at the time of admission deposit to insure registration for classes.

- Failure to submit a completed Student Health Information Form signed by a health care provider, will impact class registration.
- Letter from Student Health Center Supervising Physician to all new students about USD immunizations requirements. To avoid having a "hold" placed on registration, or to remove a "hold", a verified Student Health Information Form must be submitted.
