The University is committed to protecting the health and well-being of all our students. To achieve this goal, we recommend that all students have immunizations (MMR, Varicella, and Tetanus-Diphtheria-Pertussis) and tuberculosis screening, and REQUIRE that freshman, transfer students, international students and/or anyone living on-campus provide this information. If you are in one of the required student groups listed above, you must submit a completed Immunization & TB Verification Form. Please read the introduction letter to this form as it contains important information about how to submit the form, deadlines, holds on registration and moving into the resident halls, how the Student Health Center will communicate with you regarding receipt of complete or incomplete forms, and parental consent for those under 18 years of age.
