Waiver Process

Spring/Summer 2019 Waive Cycle

The Spring/Summer 2019 waiver form will open on December 5, 2018.

  1. Starting December 5, visit www.gallagherstudent.com/usd.
    1. Click here if you are an International Student
  2. Click on 'Enroll/Waive'
  3. Log in using your USD email address as your username and your password will be your student ID number
  4. Complete the decision form:
  5. Review the information entered for accuracy and click 'submit' to complete the process.  Immediately upon submitting your Online Form you will receive a reference number. Please note and keep this information for your record
Fall 2018 Information:

Students who did not waive before the extended September 28th deadline have been enrolled into the 2018-19 annual student health insurance plan and are responsible for the premium charged the student account.

If there were extenuating circumstances that prevented students from meeting the waiver deadline, students had an opportunity to submit a Petition to Waive after form which was accessible online from October 31, 2018 - November 5, 2018.

The Petition to Waive process acts as a second level appeal for all students who missed the published waiver deadline. Submission of a petition to waive does NOT guarantee approval.
