Eligibility

In order to receive accommodations or services, a student must self-identify and provide documentation from an appropriate licensed professional source that estalishes the existence of a disability and verifies the need for accommodations or auxiliary aids.  The documentation submitted must conform to the Documentation Guidelines set forth in this website.

USD can only provide requested accommodations after a student submits acceptable documentation to Disability Services.
