Would you like to bring a PAID performer/artist/lecturer to campus? Complete a Performance Contract and a W9 Form.

For VOLUNTEER speakers complete a Volunteer Speaker Agreement. For other VOLUNTEER service providers complete a Volunteer Services Letter.

Credit Card

If you are planning to make a purchase online or over the phone you can do so using one of the AS credit cards. To use the credit card please fill out a credit card request form and stop by SLP 308 during business hours to receive approval.
