Departments engage in many activities that have the potential to increase the University's exposure to risk. Be aware of the following issues:

Contact the Director, Risk Management at extension 7677:
- In any situation when the university's insurance coverage or liability is in question.
- Before planning a special project or event, to verify if insurance coverage is provided or is required. Please refer to the Special Events Preparation Checklist to assist you in your event planning.
- Before hiring an outside contractor or vendor who uses hazardous materials or equipment.
Contracts and Certificates:
- Any person required to sign a contract that binds the University legally, or includes any provisions for insurance coverage, should first review it with the Director, Risk Management.
- Prior to using off-campus facilities for special events, classes and seminars, the owner of the facility often requests USD supply them with a certificate of insurance naming their organization or company as additional insured. The following information must be provided to the Director, Risk Management at least 2 business days prior to the event. The information may be sent by fax to (619) 260-4061,

- Name of party to be named as "additional insured.
- Name and location of facility to be used.
- Name of facility contact person, mailing address, fax and phone numbers.
- Amount of coverage requested, if known.
- Date of event and length of time facility is to be used .
- Activity or purpose.
- Name and contact information of USD employee arranging event.
- When an independent contractor or supplier provides goods or services to the University, a certificate of insurance may be required with an attached endorsement stipulating that the University of San Diego is the additional insured. The certificate is sent to the department seeking the services of the contractor or supplier and a copy must be forwarded to the Risk Management Office. An outline of minimum insurance requirements for vendors has been developed. Contact the Risk Management office for a copy or go to Vendor Insurance Requirements.
