  1. Td (Tetanus-diphtheria, adult vaccination) or DTP (diphtheria-tetanus-pertussis, childhood vaccination): Both are 3 doses followed by a Td booster within the last 10 years.

  2. MMR (measles-mumps-rubella): 2 doses since 1968 (or positive titer)

  3. Hepatitis B: 3 doses (Must have 2 doses completed prior to entering the program then one can be completed during year 1 of the program) or positive titer

  4. Varicella (chickenpox): 2 doses (or positive titer) (will not accept note from doctor verifying chicken pox. You must have blood tested for a positive titer)

  5. TB skin test: once a year but if skin test is positive must have letter from doctor (or survey) saying that you are asymptomatic and a chest X-ray every 5 years.

  6. Certification of good health (Physical Exam): once a year (example: if you received the physical on 2/25/2004 then it would expire on 2/25/2005, you would need another before 2/25/2005) (form available via email as a Word document or in the main school of nursing office)

  7. BLS (basic life support) current certification (for healthcare providers, which includes CPR and AED for Adult/Child/Infant): renewed every 1 or 2 years. Please take the class at the American Heart Association entitled "BLS Healthcare Provider Course". Information can be accessed at http://www.americanheart.org/presenter.jhtml?identifier=3011975. This ensures that we are in compliance with regulations enforced by the facilities where clinical rotations are scheduled. (NOTE: ACLS for students enrolled in the Urgent/Emergent program)

  8. Malpractice insurance coverage: renewed yearly - Malpractice Insurance for student nurses is approximately $20.00. There are two different organizations you can use:

  9. Release of Information form - (available in the main school of nursing office or via email as a word document)

  10. California RN license (except students in the pre-licensure portion of the MEPN program): renewed every two years
